Maxillary Screw-Retained Prosthesis Supported by 12 Screw-Vent Implants

Step 1: The patient presented with an edentulous maxillary jaw that had sufficient residual bone to accommodate 12 Screw-Vent implants, 3.7mmD.

Six Months Postoperative
Step 2: All 12 Screw-Vent implants osseointegrated and soft tissue maturation was achieved with Titanium Healing Collars.

Implant-Level Impression
Step 3: Implant-level transfers were attached to each Screw-Vent implant in the patient's mouth. Full-arch, elastomeric impressions were made. After face-bow and occlusal records were made, this model was mounted with the opposing arch cast.

Laboratory Fabrication of Custom Cast Abutments and Metal Framework
Step 4: With no intraoral procedures, the laboratory fabricated custom-cast abutments, the metal substructures for single tooth restorations and a full set of provisional prostheses.

Custom Cast Abutments in Place
Step 5: After attaching the custom cast abutments to the implants in the patient's mouth, no additional preparations or retraction cord procedures were necessary to complete the case. The square core with lingual shoulder provides a perfect seat for the prosthesis, and enables the abutment to be held with pliers during attachment to minimize applied torque to the newly integrated implant.

Beautiful Provisional Restorations in Place
Step 6: The temporaries are cemented onto the custom cast abutments directly from the initial implant-level impression. No other intraoral preparations were required. The case will be completed by verifying the fit of the metal substructures, cementation of the final prostheses and adjustment of the procelain, if necessary.
